Our long-term conditions care supports whānau living with ongoing health needs such as diabetes, heart disease, respiratory conditions, and other chronic illnesses. Our approach recognises that managing these conditions requires consistent guidance, monitoring, and strong relationships.

We provide personalised care plans, regular health reviews, medication support, lifestyle coaching, and education to help you understand and manage your condition confidently. Our Nurse Practitioners work in partnership with you—promoting self-management, early intervention, and culturally grounded care that respects your goals and priorities.
